特异性尘螨皮下免疫治疗儿童变应性鼻炎的疗效
马海燕1* 王东明1 丁 玲2 郭金霞1 郑召兵1 王军霞1 1 桓台县人民医院儿科 山东 淄博 256400; 2 山东体育学院山东省科学健身指导中心 山东 济南 250102 【摘要】 目的 研究特异性尘螨皮下免疫治疗儿童变应性鼻炎( AＲ) 的疗效及对血清免疫因子和 Th1 /Th2 细胞水平的 影响。方法 回顾性分析 56 例 AＲ 患者治疗前后的临床资料，并进行视觉模拟量表( VAS) 评分，同时分别用 ELISA 和 流式细胞仪分析治疗前后血清中免疫因子 IgE、IL-2、IL-4 和 INF-γ 变化以及免疫细胞 Th1 /Th2 变化。结果 治疗前、完 成初始治疗、维持治疗 1、2 及 3 年的 AＲ 患者 VAS 评分逐渐下降。在维持治疗 2 年组患儿血清 IgE 和 IL-4 在治疗后的 水平显著下降，而 INF-γ 和 IL-2 水平显著上升，P 均 ＜ 0． 05。在维持治疗 2 年组患儿全血中 Th1 细胞含量升高，而 Th2 细胞含量降低，P 均 ＜ 0． 05。结论 特异性尘螨皮下免疫治疗能够有效缓解 AＲ 患者的临床症状，改善血清 IgE、IL-2、IL- 4 和 INF-γ 水平和恢复 Th1 /Th2 平衡。 【关键词】 过敏性鼻炎; 特异性尘螨免疫治疗; 细胞因子; Th1 /Th2 细胞; 视觉模拟量表 【中图分类号】 Ｒ725 【文献标志码】 A 【文章编号】 1001-9510( 2022) 04-0273-04 DOI: 10． 19739 /j． cnki． issn1001-9510． 2022． 04． 007
Effect of subcutaneous immunotherapy with specific dust mites on allergic rhinitis in children MA Haiyan1* WANG Dongming 1 DING Ling 2 GUO Jinxia 1 ZHENG Zhaobing 1 WANG Junxia 1 1 Department of Pediatrics，Huantai People＇s Hospital，Zibo 256400，Shandong，P． Ｒ． China; 2 Shandong Scientific Fitness Guidance Center，Shandong Sport University，Jinan 250102，Shandong，P． Ｒ． China 【Abstract】 Objective To investigate the effect of subcutaneous immunotherapy with specific dust mites on allergic rhinitis ( AＲ) in children and the levels of serum immune factors and Th1 /Th2 cells． Methods The clinical data of 82 children with AＲ before and after the treatment were retrospectively analyzed，and visual analogue scale ( VAS) score was performed． The changes of serum IgE，IL-2，IL-4 and INF-γ，as well as Th1 /Th2 of immune cells were analyzed by ELISA and flow cytometry respectively． Ｒesults VAS scores of AＲ children decreased gradually before the treatment，after the initial treatment，and 1 year，2 years and 3 years of maintenance treatment． After 2 years of maintenance treatment，the levels of serum IgE and IL-4 decreased significantly，while the levels of INF-γ and IL-2 increased significantly，P ＜ 0． 05． In the 2-year maintenance treatment group，the total blood Th1 cell content increased，while Th2 cell content decreased，both P ＜ 0． 05． Conclusion Subcutaneous immunotherapy with specific dust mites can effectively relieve symptoms of AＲ in children，improve serum IgE，IL-2，IL-4 and INF-γ levels and restore Th1 /Th2 balance． 【Keywords】 allergic rhinitis; specific dust mite immunotherapy; cytokine; Th1 /Th2 cell; blurred vision scale
