隐形矫治器对成人错颌畸形患者 ABO-OGS 评分 及牙周指数的影响
杨梦楠，罗哲 (南阳市口腔医院 口腔正畸科，河南 南阳 473000) 摘要：目的 探讨隐形矫治器对成人错颌畸形患者基于美国正畸学会客观评分系统 (ABO-OGS) 评分及牙周指数 的影响。方法 选取南阳市口腔医院 2017 年 12 月至 2019 年 12 月收治的 80 例成人错颌畸形患者，按照随机数字表法 分组，各 40 例。对照组给予直丝弓矫治器 (SWA) 治疗，试验组给予隐形矫治器治疗。统计两组 ABO-OGS 评分、口 腔咬合功能、牙周指数 [ 菌斑指数 (PLI)、牙周探诊深度 (PD)、牙龈指数 (GI)、龈沟出血指数 (SBI)]、龈沟液炎症 介质 [ 白细胞介素 -6(IL-6)、白细胞介素 -1β(IL-1β)、前列腺素 E2(PGE2)]、满意度、不良事件发生率。结果 矫治后 两组 ABO-OGS 评分比较，差异无统计学意义 (P ＞ 0.05)；矫治后试验组咀嚼次数、咀嚼效能、最大咬合时间、最大 咬合力均高于对照组，差异有统计学意义 (P ＜ 0.05)；矫治后试验组 PLI、PD、SBI、GI 低于对照组，差异有统计学 意义 (P ＜ 0.05)；矫治后试验组龈沟液 IL-6、IL-1β、PGE2 水平低于对照组，差异有统计学意义 (P ＜ 0.05)；试验组 患者满意度高于对照组，差异有统计学意义 (P ＜ 0.05)；两组不良事件发生率比较，差异无统计学意义 (P ＞ 0.05)。 结论 隐形矫治器应用于成人错颌畸形患者可取得良好矫治效果，改善牙周指数，下调龈沟液炎症介质，提高满意度。 关键词：错颌畸形；成人；隐形矫治器；直丝弓矫治器；ABO-OGS 评分；牙周指数 中图分类号：R783 文献标志码：B DOI：10.12385/j.issn.2096-1278(2022)11-0067-04
Effect of Invisible Appliance on ABO-OGS Score and Periodontal Index of Adult Malocclusion Patients YANG Mengnan, LUO Zhe (Department of Orthodontics, Nanyang Stomatological Hospital, Nanyang Henan 473000, China) Abstract: Objective To investigate the effect of the invisible appliances on the periodontal index and American Society of Orthodontics. Methods The 80 adult patients with malocclusion admitted to Nanyang Stomatological Hospital from December 2017 to December 2019 were selected and divided into groups according to the random number table method, with 40 cases in each group. The control group was treated with a straight wire arch appliance (SWA), and the test group was treated with an invisible appliance. The ABO-OGS score, oral occlusal function, periodontal index [plaque index (PLI), periodontal probing depth (PD), gingival index (GI), gingival crevicular bleeding index (SBI)], inflammatory mediators of gingival crevicular fluid [interleukin-6 (IL-6), interleukin-1β (IL-1β), prostaglandin E2 (PGE2)], satisfaction, and incidence of adverse events were statistically analyzed between the two groups. Results There was no significant difference in ABO-OGS scores between the two groups after correction (P ＞ 0.05); After treatment, the masticatory times, masticatory efficacy, maximum occlusal time, and maximum bite force of the test group were higher than those of the control group, and the differences were statistically significant (P ＜ 0.05); After treatment, PLI, PD, GI and SBI in the test group were lower than those in the control group, and the differences were statistically significant (P ＜ 0.05); After treatment, the level of IL-6, IL-1β, and PGE2 in the gingival crevicular fluid of the test group was lower than that of the control group, and the differences were statistically significant (P ＜ 0.05); The satisfaction of patients in the test group was higher than that in the control group, and the differences were statistically significant (P ＜ 0.05); There was no significant difference in the incidence of adverse events between the two groups (P ＞ 0.05). Conclusion The application of an invisible appliances in adult malocclusion patients can achieve the good orthodontic effect, improve periodontal index, reduce inflammatory mediators in gingival crevicular fluid, and improve satisfaction.
